The issue has raised due to the cache plugin as the required CSS is not loaded due to activation of the cache plugin.
So please deactivate the cache plugin and verify the issue.

ParticipantI am using W3 Total Cache. I deactivated it and tried, did not resolve the issue.

The issue is raised due to the plugin “Asset CleanUp: Page Speed Booster”, so as for now we have deactivated the plugin and the icon seem fine.
Please verify.
